Job Summary


Ensures the agency provides the highest quality of care, in compliance with federal, state, and local regulatory requirements and established departmental policies and procedures.


Duties and Responsibilities

  1. Participate in quality improvement initiatives and activities, including retrospective record review, performance improvement projects, PAC and QAPI activities. Ensures compliance with federal, state, accrediting bodies, local and agency policies in all patient care aspects.
  2. Maintain ongoing review of documentation to ensure accuracy & completeness of patient records and supervises the completion of documentation deficiencies. Works directly with staff to teach appropriate documentation practices.
  3. Serve as a resource for clinical staff and provide clinical expertise to insurance verification staff and intake coordinator for incoming referrals.
  4. Assist the Quality Director, Home & Community, in the analysis of data that includes consumer satisfaction, complaints, incidents, and reportable occurrences, quarterly chart audit results, counseling trends/concerns, training issues, infection control surveillance, and/or falls witnessed or reported.
  5. Participate in case referrals and review of initial client information related to care to be provided for home health appropriateness, anticipated care needs and assist with case assignment if needed.
  6. Contact and/or accept calls from physicians when necessary.
  7. Assist with orientation of new staff and annual education programs.
  8. Make occasional new or follow-up visits as needed.
  9. Assist with the day-to-day quality operations of the facility and other duties assigned.


Job Qualifications


Education, Training, and Licensure/Certification

  • Registered Nurse (RN) licensure in the State of Michigan required.
  • Bachelor of Science in Nursing (BSN) preferred.



Two years' community based nursing; home care or hospice preferred.


Knowledge Skills and Abilities

  • Knowledgeable of regulations covering home health.
  • Computer proficiency in Electronic Medical Records (Kinnser), Word, Excel, Power Point.
  • Ability to speak, read and write English. Excellent oral and written communication skills.
  • Excellent interpersonal skills and a team player.
  • Excellent organizational skills and ability to prioritize and problem-solve.
  • Critical thinking skills, decisive judgment and the ability to work with minimal supervision.
  • Ability to display compassion and sensitivity.


Additional Work Requirements

  • On-call availability required.
  • Ability to work flexible work hours, including evenings and weekends to support client care requirements.
  • Automobile travel required. Valid unrestricted driver's license with good driving record. Valid proof of automobile insurance.


Physical and Mental Requirements

  • Ability to lift up to fifty pounds.
  • Ability to safely lift clients, with appropriate equipment, and transport as necessary.
  • Near visual acuity.
  • Hand-eye coordination.
  • Hand and finger dexterity.
  • Ability to move freely about.
