    Job Summary

     

    Administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and Plan of Care under the direction and supervision of the Director of Clinical Services/Administrator.

     

    Duties and Responsibilities

    1. Conduct initial evaluation visit and regularly reevaluates the patient's nursing needs.
    2. Provide services in accordance with the Plan of Care and make necessary revisions using appropriate preventive and rehabilitative nursing procedures. Conduct patient care conferences on patients assigned to his/her care.
    3. Prepare clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per policy.
    4. Coordinate interdisciplinary services for patient to ensure appropriate care. Refer to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.
    5. Counsel the patient and family/significant others in meeting nursing and related needs.
    6. Process orders and notify physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.
    7. Delegate care appropriately and supervises other licensed and unlicensed personnel within appropriate scope. Participate in peer review and performance improvement as assigned.
    8. Participate in utilization review of medical records as assigned.
    9. Complete and submit OASIS assessments, reassessments, and transfers, resumptions of care, discharges and significant change in condition, clinical documentation in accordance with Agency defined time frames. Appropriately utilizes ICD-9/ICD-10 codes.

    Job Qualifications

     

    Education, Training, and Licensure/Certification

    • Current Registered Nurse (RN) licensure for state of practice.

     

    Experience

    • Two years' experience as a Registered Nurse; home health care experienced preferred.

     

    Knowledge Skills and Abilities

    • Ability to exercise initiative and independent judgment.
    • Ability to display compassion and sensitivity with vulnerable adults.
    • Excellent communication skills with the ability to read, speak and write English using proper grammar, diction and style.
    • Ability and willingness to work constructively with individuals from diverse cultural backgrounds.
    • Working knowledge of computer software including Microsoft Office Word, Outlook, Excel and Electronic Medical Records (EMR) programs.
    • Superior customer service aptitude and approach.

     

    Additional Work Requirements

    • Required to assume on-call duty nights, weekends and holidays, as assigned.
    • Valid Michigan driver's license; reliable personal transportation; current proof of automobile insurance.
    • In-service and other training attendance required.

     

    Physical and Mental Requirements

    • Frequently stoops, twists, bends, squats and kneels.
    • Lifts overhead.
    • Frequently lifts supplies weighing 25 pounds.
    • Feels pulse rate and hears and reads blood pressure.
    • Needs to see and hear persons served.
