INTAKE ASSESSMENT CLINICIAN - HOLLAND, MICHIGAN

 

Do you have a Master’s degree in social work, counseling, or a related field and a LLMSW, LMSW, LLPC or LPC? We have a new career opportunity for a Substance Use Disorders Intake Assessment Clinician, collaborating with our team in our Holland offices.

 

If you have experience working with Substance Use Disorders clientele, this integral member of our interdisciplinary team responds to all requests for substance use disorder services by completing intake, assessment, and determining the appropriate level of care. The Intake/Assessment Clinician works with internal and external partners to ensure referrals made to programs are quickly assessed and provide a welcoming initiation to Samaritas programs.

This position has a pay range of $55,000 - $64,000 BASED ON APPLICABLE EXPERIENCE. Benefits are effective the first of the month following 30 days of employment with some details listed below.

The Intake/Assessment Clinician responds to all requests for substance use disorder services by completing intake, assessment, and determining the appropriate level of care. The Intake/Assessment Clinician works with internal and external partners to ensure referrals made to programs are quickly assessed and provide a welcoming initiation to Samaritas programs.

 

Duties and Responsibilities

  1. Promptly responds to requests for services received.
  2. Accurately completes intake documentation and gathers pertinent collateral information for each referral.
  3. Follow-up with client and referral sources to ensure needs are met.
  4. Conducting clinical interview/assessment, determining diagnosis based on the criteria outlined in the latest version of the Diagnostic and Statistical Manual (DSM) and clinical appropriateness for services.
  5. Regularly obtain consultation to determine clinical appropriateness of potential clients.
  6. Facilitate client admission into services by explaining level of care, program recommendations, and ensure understanding of upcoming service appointments.
  7. As needed, make referrals for outside health providers, support groups, and/or social service agencies.
  8. Seek and obtain pertinent information regarding the financial eligibility of potential clients, consulting with Administrative Coordinators, Program Supervisors, Financial Department, and Program Director, as necessary.
  9. Complete timely documentation of appointments, clinical assessments, documentation, and contacts internally and externally.

Job Qualifications 

 

Education, Training, and Licensure/Certification 

  • Master’s degree in social work, counseling, or a related field, from an accredited institution.
  • Must have LLMSW, LMSW, LLPC or LPC

    Experience 
  • Minimum of one year Substance Use Disorder SUD counseling experience preferred.
  • Familiarity with SUD documentation preferred.
  • Two years of clinical experience working with (SUD) clientele preferred.
  • Prior experience working with an interdisciplinary team preferred.

 

Knowledge Skills and Abilities 

  • Ability to apply common sense and understanding to carry out instructions furnished in written, oral, or diagrammatic forms.
  • Ability to work constructively with supervisor and staff.
  • Ability to learn how to utilize technology through computer and phone apps.
  • Ability to maintain confidentiality with client, employee and organization information.
  • Ability and willingness to work as part of a team in achieving common goals.
  • Ability to work constructively with staff and other stakeholders.
  • Ability to work effectively in a cross-cultural context.
  • Ability to speak, read and write fluent English. Strong oral and written communication skills with ability to prepare letters, summaries and reports using prescribed format and conforming to rules of punctuation, grammar, diction, and style.
  • Ability to manage multiple functions of a complex organization.  Strong organizational skills.

  • Valid unrestricted driver's license with good driving record and proof of automobile insurance.
