Provides case management services for eligible families transitioning from the Family Center to affordable housing, in accordance with all applicable contractual and regulatory requirements and accreditation standards.

 

Job Expectations (Essential Functions, Specific Duties and Responsibilities):

 

  1. Provide comprehensive, strength-based case management services to all assigned residents including intake assessment, goal-setting, progress monitoring, financial literacy, resources and referrals and advocacy.
  2. Conduct assigned residents initial assessment within required time frame, ensuring complete, accurate and timely case notes, record-keeping, reports and documentation.
  3. Maintain accurate and timely submission of residents entry, exit and updates in the Homeless Management Information System (HMIS)
  4. Develop and implement family engagement strategies that establish working partnerships with residents. Create a housing plan to transition residents from the Family Center to stable and affordable housing; address the barriers to obtaining and maintaining housing.
  5. Educate residents on referrals/services available and the procedures for applying for/accessing referrals/services, as well as their rights related to services. Assist residents in completing required documentation. Monitor housing plan progress. Conduct weekly follow-up sessions with residents and referral agencies to ensure residents are implementing housing plans, and utilizing resources and referrals in a timely manner. Conduct meetings with families every 30 days to assess continuation at the Family Center.
  6. Participate in in-house case consultations and meetings, facilitating problem-solving, resource- sharing, and communication regarding residents. Address non-compliance issues with Family Center Rules. Support and guide families in positive decision-making.
  7. Provide monthly aftercare as required to ensure a smooth transition from the Family Center into affordable housing and local community; support residents in independent living to prevent reoccurrence of homelessness. Will require some travel to new homes
  8. Serve as an advocate for residents, and act as their liaison with service providers and community agencies and implement strategies to acknowledge and/or celebrate resident achievements and accomplishments. Implement financial literacy programming to guide residents in making sound decisions regarding their personal finances. Facilitate ongoing communication with KidZone’s early education and care staff members in order to assess assigned children's progress, as well as other family needs.
  9. Represent the Family Center and the organization at fairs, community events and outreach efforts, promoting community awareness of the Family Center and its services.

Job Qualifications:

 

Education, Training, and Licensure/Certification

 

  • Bachelor degree in social work, psychology, or related human services field required

 

Experience

 

  • One year of experience in human or community services preferred.
  • Previous experience working with adults or families in crisis preferred.

 

Knowledge Skills and Abilities

 

  • Strong problem resolutions skills. Ability to work with minimal supervision.
  • Ability to add, subtract, multiply and divide all units of measure.
  • Ability to read, speak and legibly write English with proper use of pronunciation, format, punctuation, spelling and grammar.
  • Ability to relate with staff, representatives of HUD, landlords and representatives from community organizations.
  • Ability and willingness to display compassion and sensitivity to families.
  • Working knowledge of computers including Microsoft Office, the Internet, and job related software such as HMIS.
  • Knowledge of local community and housing resources.
  • Ability to maintain confidentiality of resident information.
  • Ability to assist others in developing goals and helping hold them accountable to achieve those goals
  • Unrestricted driver's license with good driving record. Reliable personal transportation suitable for safely transporting persons served; proof of current automobile insurance.
